Our form asks for your consent for things such as having photographs taken of your child, their use of the Internet, visits to the local area and use of the library system thumb scanner if required.
Please advise us of any medical matters that we need to know about to help us look after your child.
Earrings should not be worn for PE due to safety concerns but if you want our child to keep their earrings in then please complete this form.
